Applicants for naturalization seeking an exception to the English and/or civics requirements for naturalization because of a physical or developmental disability or mental impairment must submit this form when they file Form N-400 Application for Naturalization, with U.S. Citizenship and Immigration Services (USCIS). USCIS generally only considers a Form N-648 that is concurrently submitted with a Form N-400.
