 Dr. Olga Aleksandrova M.D is a designated  physician, also known as a civil surgeon, who is authorized by U.S. Citizenship and Immigration Services (USCIS) to perform medical exam required for Green Card applicants or adjustment of status applicants.

What to bring to exam?


  • A government-issued photo identification document (passport /green card/driver’s license),document containing your Alien/USCIS number


  • If available, please bring any copies of foreign or domestic immunization and vaccination records. If you do not have it, NO PROBLEM. We will provide you with blood work order to figure out your antibody levels for the required vaccines


  • I-693 Form (Report of Medical Examination and Vaccination Record). We will send you electronic form for I-693.




Required specific tests
the day of physical 


  • Testing for Tuberculosis - All applicants 2 years old or older must have an interferon gamma release assay (IGRA)- Blood. If positive must have a chest radiograph (chest x-ray)


  • Testing for Syphilis- All applicants aged 15 and older


  • Testing for Gonorrhea- All applicants 15 years of age & older

No immunization records? 

  • Laboratory evidence of immunity is acceptable for measles, mumps, rubella, hepatitis A, hepatitis B, polio, and varicella if the applicant lacks acceptable documented history of vaccination for these diseases. We will order titers - blood test to  check your immunity for those infections

  • If your titers comes back that you do not have any immunity, meaning you never were sick or were vaccinated, we will refer you to the department of health to complete your vaccinations.

 if you have insurance you can use your insurance

to check your immunization status  or for vaccine injection 

Covid Shot is free at any pharmacy. 

Validity Period of Form I-693

​After the benefit application was filed with USCIS, Form I-693  retains its evidentiary value for 2 years from the date of the civil surgeon’s signature.
